Payment is expected upon services rendered.Dr. Moore is not a participating provider with any insurance company, but the office will submit to most insurance plans.We will submit insurance claims as a service to the client, but reserve the right to bill for the remaining balance not paid by insurance. To find out if your insurance provider covers our services, please contact your insurance company. We do not submit claims to Medicare, Medicaid, or any other goverment funded insurance.We accept major credit cards including, Visa, Mastercard, and Discover.
